Please complete the following questions for appointed staff and Postdoctoral Fellows only. Appointed staff include Faculty Members who are paid through the University and have their primary appointment in Physiology, Research Associates, Lab Technicians (USW Appointed, ie. not casuals), and Administrative Staff (USW Appointed, ie. not casuals). This site is secure and information submitted here will only be used by the Departmental Business Manager to update personnel records in the University's Human Resources Information System (HRIS).

Enter the first day of absence (Start Date) and the last day of absence (End Date). If the employee has not yet returned to work, please enter today's date in the End Date text box. For single-day absences (ie. not a range), please enter the absence day(s) under the 'Single Days' section.
